Starting with the 2019-20 school year, students in Jersey Community Unit School District 100 who need the required physical and immunizations must submit them before the first day of school. Students who have not met the mandated requirements will not be allowed to attend school until the records are turned in. In order to assist parents in preparation for the 2019-20 school year, JCUSD No. 100 will be hosting school based clinics. The Physical and Immunization Clinics are being offered by the JCH Medical Group and Jersey County Health Department. Tdap and meningococcal vaccines will be available for students entering 6th grade, and meningococcal vaccines will be offered for students entering 12th grade. Mandated Kindergarten, 6th and 9th grade physicals, as well as sports physicals will also be offered.
This is an excellent opportunity to get ready for next school year. Participants will need to have a signed permission slip and the appropriate forms completed. They should be returned to the school office by May 3, 2019. Parents do not have to be present, but permission slips must be signed. The cost for a physical is $25 cash or check. No insurance will be billed. The fee can be returned with the permission slip or on clinic day. It must be paid in order to participate. Permission slips, meningitis and Tdap vaccine information sheets and screening checklists can be found on-line at www.jersey100.org or in the school office. The State of Illinois physical forms for sports and mandated kindergarten, 6th and 9th grades are also available online.
